‘Vital’ services coming to an end August 29

The Oklahoma State Department of Health has announced the Garvin County Health Department will discontinue its service of providing death certificates effective close of business on Aug. 29.

“We are discontinuing this services due to the high cost of required system upgrades and the low volume of requests historically received by this office,” said Center for Health Statistics Director Kelly Baker.

“I am very appreciative of Mike Milton and his staff at the Garvin County Health Department for providing outstanding Vital Records service to the public for more than 10 years.”

Birth and death certificates continue to be available by submitting an application in person at the Oklahoma City, McAlester and Tulsa area health department offices.

Also, requests can be mailed to the Oklahoma City office: Oklahoma State Department of Health, Division of Vital Records, 1000 N.E. 10th Street, Oklahoma City, OK 73117.

Expedited services may be requested on-line at www.VitalChek.com for an additional fee. Specific requirements to obtain a certificate can be found on-line at http://vr.health.ok.gov.

To apply in person, the following Vital Records office locations are open Monday through Friday from 8:30 a.m. until 4 p.m.:

• Tulsa City County Health Department, Central Regional Health Center, 315 S. Utica, Tulsa, OK 74104; Telephone - 918/594-4840.

• Pittsburg County Health Department, 1400 East College Avenue, McAlester, OK 74501; Telephone - 918/423-1267.
